The course of the Douglas Water is entirely within the South Lanarkshire council area.

The river originates in the mountains that separate Lanarkshire from Ayrshire, South-West of Muirkirk. A source close to the river Ayr, which flows West to the sea, but the Douglas water flows North-East, then East, past the Glespin and Douglasdale. Here the river flows through the village of Douglas, and the last meager remains of Douglas castle. The castle was the stronghold of the house of Douglas, a powerful medieval family, who took their surname from the river. On the A70 road follows the river through Douglasdale and Clyde.

East of Douglas, the river flows on the M74 motorway, just South of the Happendon services, and turns to the northeast again. Below the village of Rigside, the less Poniel burn flows into the Douglas water. In addition, the merger is the small village of Douglas water. Dismantled railway line runs between the river and the A70 for the remaining 2 km 3 km the course. Douglas water flows into the Clyde, about 3 MI 5 km South of Lanark.
